Welcome to Forma Aesthetics

Hi! Our names are Dominique & Beth, and we are the co-partners and faces behind Forma Aesthetics! We are excited to offer Medical Aesthetics services to Brooks, AB and surrounding areas! Here is a little bit about us:

✨ We are nurses who met working together in the Brooks emergency room, but we’re both born and raised in Brooks. We share a passion for improving health, wellness and beauty.

✨ Beth is a Nurse Practitioner and Dominique is a Registered Nurse, making us an ideal team for this industry. See our individual work experience in our bios!

✨ We take pride in evidence-based information and adhering to best practices in the world of aesthetics.

✨ With Beth acting as Medical Director and Dominique as the primary Nurse Injector, you can trust you are in good hands and that we can safely offer you a wide range of services to address your beauty and wellness needs!

Consultations

Unsure what treatment is best for you or want to learn more about our services? Book a complimentary consultation with us. We also offer consultations specifically for skin assessment, therapeutic neuromodulator, and dermal filler.

Wrinkle Relaxers

Wrinkle relaxers, such as Botox® and Dysport®, are used to relax facial muscles and reduce the appearance of fine lines and wrinkles. They can also be used for therapeutic purposes, such as for chronic migraine management, excessive sweating, or TMJ pain.
Pricing: $9/unit of Botox®/Dysport®

Dermal Fillers

Dermal filler is used to augment certain areas of the face and restore lost volume. We use the Revanesse brand and currently offer fillers for the lips, chin, lateral cheeks, nasolabial folds and marionette lines.

Pricing: $499/full syringe | $299/half syringe

Facials

We currently offer two different facial treatments: chemical peels and hydrating facials with pharmaceutical grade Vivier® skincare products.
Pricing: $126/chemical peel | $75/hydrating facial

Therapeutic Injections

Book here for Therapeutic Botox. Botox has many uses outside of the cosmetic world - we offer Botox for excessive sweating (hyperhidrosis), TMJ pain, and chronic migraine management. If you think you may be a candidate for any of these treatments, book a complimentary consultation with our Nurse Practitioner. Text 403-501-8053 to request an appointment.

Pricing: $150 injection fee/session

Dominique Grundy RN

✨ Dominique completed her Bachelor’s of Science in Nursing. She’s born and raised in Brooks, Alberta. You’ve probably seen her around the Brooks Hospital, or down at her mom’s hair salon among many other spots in the community! Dominique loves making a difference in people’s lives so nursing was a perfect career choice! In addition, she has always had an eye for fashion, beauty and aesthetics – so being able to blend these two passions is what drove her into the world of medical aesthetics!

Her patient care experience, education, and training under @drandrewdargie & @thebotoxcourse will ensure the highest level of safety, evidence-based practice, and care. Beth Motokado NP

✨ Beth Completed her Master’s of Nursing – Nurse Practitioner program through Western University in London, Ontario in 2018. Since then, she has spent most of her time working in a busy family practice for complex care clients in Calgary. Beth began her journey into aesthetic medicine after learning about and seeing the benefits medical aesthetics can provide to people’s self-confidence and quality of life. Beth is passionate about helping people feel their best – so co-starting Forma Aesthetics was a dream come true!

Facial anatomy is a complex area, and you want to ensure your treatments are carried out by professionals who have completed extensive training. Fun fact – Beth has worked alongside oral maxillofacial surgeons assisting in ORs in Calgary where she’s assisted in orthognathic jaw surgeries, and provided therapeutic treatments for TMJ syndromes.

When she’s not working, you can find her out with her bernedoodle Oakley, and Shaun enjoying dog parks, mountain hikes, or camping with a coffee in her hands 💋
